This Weight-Loss Consumer Bill of Rights and Important Program Disclosures page supplements the Terms and Conditions, Privacy Policy, Notice of Privacy Practices, telehealth consent documents, treatment consents, and other policies governing the use of the Healthy Path Rx website, platform, and related services.

This notice applies to patients and prospective patients participating in weight-management, metabolic-health, nutrition, wellness, and medication-assisted weight-loss programs made available through Healthy Path Rx and participating licensed healthcare providers.

This page is provided for informational and compliance purposes. Viewing or using this page does not, by itself, constitute informed consent to medical treatment. Patients may be required to complete separate telehealth, treatment, medication, pharmacy, and financial acknowledgments before receiving services.

I. Florida Weight-Loss Consumer Bill of Rights

The following Weight-Loss Consumer Bill of Rights is provided to Florida consumers pursuant to section 501.0575, Florida Statutes.

Warning: Rapid Weight Loss May Cause Serious Health Problems.

Rapid Weight Loss Is Weight Loss Of More Than 1 1/2 Pounds To 2 Pounds Per Week Or Weight Loss Of More Than 1 Percent Of Body Weight Per Week After The Second Week Of Participation In A Weight-Loss Program.

Consult Your Personal Physician Before Starting Any Weight-Loss Program.

Only Permanent Lifestyle Changes, Such As Making Healthful Food Choices And Increasing Physical Activity, Promote Long-Term Weight Loss.

Qualifications Of This Provider Are Available Upon Request.

You Have A Right To:

  1. Ask Questions About The Potential Health Risks Of This Program And Its Nutritional Content, Psychological Support, And Educational Components.
  2. Receive An Itemized Statement Of The Actual Or Estimated Price Of The Weight-Loss Program, Including Extra Products, Services, Supplements, Examinations, And Laboratory Tests.

I. Additional Patient Rights

Patients participating in services made available through Healthy Path Rx have the right to:

  • Ask questions about any proposed weight-management program, treatment plan, medication, supplement, laboratory test, nutritional recommendation, educational material, or related service.
  • Receive information about the potential benefits, risks, side effects, contraindications, alternatives, and limitations associated with a proposed treatment.
  • Receive a written itemized statement of the actual or estimated self-pay cost of the recommended program, including, where applicable, consultations, medications, pharmacy charges, laboratory testing, supplements, memberships, subscription fees, examinations, shipping, and ancillary services.
  • Receive information regarding the actual or estimated duration of the recommended program, recognizing that the duration may change based on medical circumstances, patient response, treatment adherence, medication availability, or the independent clinical judgment of the treating provider.
  • Request information regarding the identity, licensure, credentials, and qualifications of the licensed healthcare professionals involved in evaluating, approving, prescribing, recommending, or managing treatment.
  • Receive information in a manner that is reasonably understandable and have an opportunity to ask questions before making healthcare decisions.
  • Participate in decisions concerning their care and refuse or discontinue treatment to the extent permitted by law.
  • Be treated with dignity, respect, and professionalism and without unlawful

I. General Weight-Management and Wellness Disclosures

Weight-management treatment is individualized. Treatment recommendations may vary based on a patient’s age, medical history, current medications, allergies, laboratory results, weight, body composition, treatment goals, pregnancy status, risk factors, prior treatment response, and other relevant clinical information.

Patients understand and acknowledge that:

  • Weight-management programs may include nutrition and lifestyle recommendations, physical activity, behavioral modifications, laboratory testing, prescription medications, nonprescription products, supplements, or other provider-directed interventions.
  • Prescription medications and other treatments may carry risks, side effects, contraindications, limitations, and potential interactions with other medications or medical conditions.
  • No treatment is risk-free, and both common and uncommon adverse effects may
  • Serious adverse events, although uncommon, may require treatment discontinuation, urgent evaluation, emergency care, hospitalization, or additional medical treatment.
  • Remaining overweight or obese may also be associated with health risks, including diabetes, hypertension, cardiovascular disease, sleep apnea, joint disorders, metabolic disease, and other health complications.
  • Alternative approaches may include lifestyle modification, nutrition counseling, behavioral support, exercise programs, treatment through a personal physician, FDA-approved medication, bariatric evaluation, or other medical or nonmedical
  • Results vary between Healthy Path Rx and participating healthcare providers do not guarantee weight loss, a particular rate of weight loss, maintenance of weight loss, medical improvement, or treatment success.
  • Long-term results generally require continued attention to nutrition, physical activity, behavior, sleep, medical conditions, and other lifestyle factors.
  • Weight regain may occur after medication reduction or
  • Continued participation may require follow-up visits, laboratory testing, vital-sign monitoring, medication reconciliation, medical evaluations, and compliance with provider recommendations.

I. Prescription Medication and Off-Label Treatment Disclosure

All prescriptions are issued only when a licensed healthcare provider determines, in the

provider’s independent professional judgment, that a medication is medically appropriate for the patient.

The patient understands that:

  • Prescription medications may have known and unknown risks, side effects, precautions, contraindications, and drug interactions.
  • Depending on the medication prescribed, potential risks may include nausea, vomiting, diarrhea, constipation, abdominal discomfort, dehydration, fatigue, dizziness, headaches, allergic reactions, injection-site reactions, elevated heart rate, blood-pressure changes, gallbladder complications, pancreatitis, changes in blood sugar, medication intolerance, mood or behavioral changes, and other medication-specific risks.
  • Certain medications may not be appropriate for patients who are pregnant, attempting to become pregnant, breastfeeding, or who have particular medical conditions, medication interactions, allergies, or personal or family medical histories.
  • Patients will receive medication-specific instructions and should review all medication guides, pharmacy labels, warnings, storage instructions, and administration directions supplied with their medication.
  • A licensed provider may prescribe an FDA-approved medication for a use, treatment duration, dosing schedule, or titration schedule that differs from the medication’s FDA-approved labeling when the provider determines that such use is medically
  • Off-label prescribing does not mean that the medication is experimental or illegal. However, the particular use, duration, dose, or schedule may not have been evaluated or approved by FDA to the same extent as the uses described in the medication’s approved
  • Patients may ask their provider whether a recommended treatment is FDA approved for the proposed use or is being prescribed off-label.
  • Medication availability, pharmacy fulfillment, dosing, and continued prescribing are not guaranteed and may change based on clinical circumstances, applicable law, regulatory requirements, product availability, or pharmacy policies.

I. Compounded Medication Disclosure

This section applies only when a compounded medication is prescribed.

A compounded medication is prepared for an individual patient by an appropriately licensed compounding pharmacy or other legally authorized compounder pursuant to a valid prescription and applicable law.

The patient understands and acknowledges that:

  • Compounded medications are not FDA
  • FDA does not evaluate or verify the safety, effectiveness, quality, manufacturing consistency, or therapeutic equivalence of a compounded medication before it is dispensed to a patient.
  • A compounded medication is not the same as an FDA-approved generic drug and should not be assumed to be identical, therapeutically equivalent, or interchangeable with an FDA-approved brand-name or generic product.
  • Compounded medications may be appropriate when a licensed provider determines that a patient has an individualized medical need that cannot be met by a commercially available FDA-approved medication and when compounding is permitted by applicable
  • The appearance, concentration, ingredients, formulation, vial size, syringe instructions, storage requirements, beyond-use date, or packaging of a compounded medication may differ from another compounded product or an FDA-approved medication.
  • Injectable compounded medications may be supplied in different Patients must follow the exact milligram, milliliter, and syringe-unit instructions provided for the specific medication dispensed to them.
  • Patients must not use instructions from a previous prescription, another patient, social media, an online calculator, or a different medication vial.

  • Patients must contact the prescribing provider or dispensing pharmacy before administering a dose if the label, concentration, syringe instructions, appearance, storage condition, or prescribed amount is unclear.
  • Patients should not use medication that arrives damaged, improperly sealed, incorrectly labeled, excessively warm when refrigeration is required, visibly contaminated, or otherwise inconsistent with the pharmacy’s instructions.
  • A prescription for a compounded medication may be changed, delayed, transferred, or discontinued if the provider or pharmacy determines that continued compounding is no longer clinically appropriate or legally permissible.

Compounded medications will be prescribed only when the treating provider determines that their use is clinically and legally appropriate for the individual patient.

I. Telehealth Services

Healthy Path Rx facilitates access to healthcare services delivered through telehealth. The
specific licensed healthcare provider or medical group responsible for a patient’s medical care will be identified during the care process.

Telehealth may involve electronic communications, online health questionnaires, secure messaging, telephone calls, video visits, remote monitoring, electronic prescribing, and the electronic exchange of medical information.

Patients understand that:

  • Telehealth has limitations and may not allow a provider to perform every examination, measurement, diagnostic test, or assessment that could be performed during an in-person
  • A provider may require an in-person examination, laboratory test, diagnostic study, specialist consultation, or other evaluation before initiating or continuing
  • Technology failures, internet interruptions, delayed communications, unauthorized access, or other technical and privacy risks may occur.
  • Patients must provide complete, current, and accurate information and must identify their physical location at the time of a telehealth encounter when requested.
  • Patients may be asked to verify their identity and provide government-issued identification or other identifying information.
  • Patients should participate from a private, safe location and must not participate in a telehealth encounter while driving or operating machinery.
  • Telehealth services are not appropriate for every medical condition or every
  • A patient may decline or withdraw from telehealth services, subject to applicable law and any limitations necessary to safely transition or discontinue care.
  • Telehealth encounters and related communications may be documented in the patient’s medical record.

Separate telehealth consent may be required during patient onboarding.

I. Independent Medical Judgment

Medical services made available through Healthy Path Rx are provided by licensed healthcare professionals and, where applicable, independent professional medical groups.

All diagnoses, prescriptions, treatment recommendations, medical evaluations, eligibility decisions, and determinations regarding whether treatment should be initiated, modified, continued, or discontinued are made solely by the treating licensed healthcare provider exercising independent professional medical judgment.

Healthy Path Rx does not guarantee that a provider will prescribe any particular medication or approve any requested treatment. Healthy Path Rx does not direct or interfere with a provider’s independent clinical judgment.

The provider-patient relationship is between the patient and the licensed healthcare provider or medical group responsible for the patient’s care. The identity of the responsible provider or medical group will be disclosed during the care process.

I. Self-Pay Services and Insurance Reimbursement

Unless expressly stated otherwise in writing, services purchased through Healthy Path Rx are offered on a self-pay basis.

Patients understand and acknowledge that:

  • Healthy Path Rx generally does not submit insurance claims on the patient’s behalf for services purchased through the platform.
  • Healthy Path Rx does not submit claims to Medicare, Medicaid, TRICARE, or other federal or state government healthcare programs for services purchased through the platform unless expressly stated otherwise in writing.
  • Patients are responsible for all applicable consultation fees, program fees, membership or subscription charges, laboratory charges, medication and pharmacy charges, supplements, shipping costs, and ancillary-service fees.
  • Before enrollment or treatment, patients will be provided with applicable pricing information, including a written itemized statement or estimate of the cost of the recommended program and its actual or estimated duration when required.
  • Patients who are uninsured or are not using insurance may be entitled to a Good Faith Estimate of expected charges under applicable federal law.
  • Estimates are based on the information available when Actual charges may change if the patient’s treatment plan, medication, laboratory needs, pharmacy, dosage, duration, clinical condition, or requested services change.
  • A pharmacy, laboratory, medical group, or other third party may charge the patient
  • Healthy Path Rx does not guarantee that any service, medication, laboratory test, supplement, membership fee, or other expense will qualify for reimbursement through private insurance, an employer benefit, a health savings account, a flexible spending account, or another third-party benefit arrangement.
  • Patients are responsible for reviewing applicable cancellation, refund, recurring-billing, medication-replacement, and shipping policies before purchasing services.

I. Patient Responsibilities

Patients are responsible for actively participating in their care and agree to:

  • Provide complete, truthful, current, and accurate medical
  • Disclose all medications, supplements, allergies, medical conditions, prior surgeries, eating-disorder history, mental-health history, substance use, pregnancy, breastfeeding, attempts to become pregnant, and other information requested by the provider.
  • Inform the provider promptly of changes in medical history, medications, symptoms, pregnancy status, or other relevant circumstances.
  • Follow the exact dosing, titration, administration, storage, and monitoring instructions provided by the treating provider and dispensing pharmacy.
  • Never share prescription medication, needles, syringes, or other prescription supplies with another person.
  • Never increase a dose, shorten the interval between doses, restart medication after an extended interruption, or combine treatments without provider approval.
  • Attend required follow-up visits and complete required laboratory testing or medical
  • Promptly report side effects, medication errors, unexpected symptoms, possible pregnancy, allergic reactions, worsening medical conditions, or concerns about
  • Seek urgent or emergency care when advised or when symptoms may represent a medical

I. Emergencies and Urgent Medical Concerns

Healthy Path Rx is not an emergency medical service.

Patients experiencing a medical emergency—including severe abdominal pain, persistent vomiting, inability to keep fluids down, signs of severe dehydration, difficulty breathing, facial or throat swelling, chest pain, fainting, confusion, severe weakness, symptoms of a serious allergic reaction, thoughts of self-harm, or another potentially life-threatening condition—should call 911 or go to the nearest emergency department.

Patients should not rely on website messages, email, text messages, or routine telehealth communications for emergency assistance.

For nonemergency but urgent medication questions, patients should contact their treating provider, the dispensing pharmacy, or Healthy Path Rx Patient Support using the contact information provided to them.

I. Informational Notice and Patient Acknowledgment

This webpage is intended to provide patients and prospective patients with important information regarding their rights and participation in weight-management services.

Accessing or using the Healthy Path Rx website does not, by itself, establish a provider-patient relationship or demonstrate informed consent to a particular medical treatment.

Before receiving medical services, patients may be required to affirmatively acknowledge or electronically sign separate documents addressing:
